Event Request Form

Please fill out the following form if you are:

- Scheduling a Christ Church event as a staff member or parishioner

- A non-Christ Church entity, requesting use of the facility for a one-time or ongoing special event

-Before filling out the form, please check the Building Use Calendar to make sure there are no conflicts.
